Orientation ui button and custom orientation creation improvements

A. I'd prefer if the buttons were all available side by side and not in drop down menu. So you can switch between things with single mouse click intead of two plus having to browser through the list. As this button layout has no place for the text there should be mouse hover over tooltip that shows the names (glogal, local....). Like the vert/line/face menu.

B. Add a button for creating new orientations. Currently this feature is hidden away and not a lot of people even know about it. I'd just add a [+] button next to these other buttons in the ui. When you want to add a new orientation you just select your verts and press that button and get a popup menu asking for name and confirm.

rant: In 2.8 there is a +button there but it almost looks like a typo as it doesn't convey any functionality unless you know beforehand what it does. The tool also works unintuitively as it acts like a tool but requires a selection before the tool can be succesfully activated. It should be the other way around. Press button, make selection and then name it and press ok to finish the tool. Or grey out the button unless a suitable selection is active.
