make "Align active camera to view" create a camera if there is none
This has been a papercut to me forever. Starting with an empty scene, modeling something, getting the view right, pressing alt ctrl num0 but there is no camera. so I have to leave my selection, switch to obj.mode, add a camera and back again and where was I ?? doesn't happen often but sucks when it does. So if Blender could just add a camera to the scene in case of no camera - that would be great, because it is obviously what the user wants. Or maybe I am just lazy and unorganized...

Intelligent prediction behavior is a great UX thing to implement. Modo uses a methodology of predicting what the user wants and acts accordingly, taking out a lot of manual task work and mode switching. If you can think of any more of these types of "auto behaviors", I think Blender could certainly benefit from them.
