Inset distance independent of cursor location
When using Inset while the mouse cursor is inside of the face the distance you can inset the face is limited by the distance the mouse cursor is away from the center of the face, at the time of activating the tool. In the worst case scenario (when the mouse cursor is in the exact center of the face) you can only inset a very small distance, or not at all. My suggestion would be to always start the inset from the position of the mouse, where's movement to the left would inset the face and movement to the right would "outset" it, regardless if the mouse cursor is inside the face or anywhere else.
Edit: The GIF didn't work so I uploaded a screenshot.

yessss. this bugs me so much. the first time i ran across it i thought it was a bug and it was closed on devtalk as "working as intended". apparently this is something to do with how modal operators work. it should definitely be fixed, whatever the reason!
Yes, and it should also work with scaling
