EEVEE live in a seperate output window to capture in OBS
Is It possible to have the EEVEE output in a separate window (effectively without clicking render) and without any GUI so it can be captured by OBS studio, or it's like, in real-time.
I'm manipulating blender things in 3D via MIDI and this would make real-time output capture for digital puppeteering

Yes, this is possible. You would go to the Window menu, Choose New Window, set the window to Look Dev or Render (EEVEE) Mode, shut off Overlays and shut off Gizmos, then hit Ctrl+Alt+Space to make it "full screen". You would still have the title bar to deal with, but that could be cropped in post.
you could also record the animation in blender and render it accordingly later , if video is your endproduct anyways that should work
